The Emily Center at Phoenix Children’s Hospital has developed the Our Journey™ with a Stem Cell Transplant tool, addressing the critical needs of families navigating the complexities of post-hospital care for children undergoing stem cell transplants. This resource aims to empower families with the knowledge necessary for a safe and confident transition home.

Families often find themselves inundated with medical terminology from healthcare professionals, which can obscure understanding and lead to confusion. It is not uncommon for providers to assume clarity without verifying that parents grasp their instructions fully. The ability to articulate information in one's own words is a vital indicator of comprehension.

The Our Journey™ with Stem Cell Transplant app serves several important functions:

• It facilitates open dialogue between families and the healthcare team.
• It assists parents in identifying essential knowledge required for effectively caring for their child at home.
• It provides a supportive environment for parents to express uncertainty and seek clarification on topics they do not understand.
• It prompts families on key concepts and skills they should demonstrate back to their healthcare providers, allowing for clarification and correction of any misunderstandings.
• It aims to instill confidence in families regarding their ability to care for their child after discharge, recognize potential complications, and respond appropriately.
